The social work team at Sydney Hospital and Sydney Eye Hospital provide the following services:

  • psychosocial assessments
  • accommodation advice and organisation of appropriate placement in supported accommodation
  • counselling for patients, carers and families
  • discharge planning and liaison with community services eg Centrelink, housing services, community health teams, aged care services and non-government organisations
  • ongoing close liaison with services for the homeless of the inner city

The hospital Social Work team are available Monday to Friday, 8.30am - 5pm, and can be reached after hours or on weekends if necessary. All voice mail messages are promptly responded to.
